CLINICAL TRAILS ON MIGRAINE

INTRODUCTION -

Third most common disease in the world that comes under neurological disorders of the brain between the ages of 18-44 years, Where 85%  of women are suffering from chronic migraines (suffers 15 days in a month with  migraine pain).Many migraine sufferers are not able to work normally as others during migraine. Girls are affected more than boys in the ratio of (4:1) but boys affect migraine before the puberty. Till now there is no preventive medication for the migraine, this treatment can be done by specialists.  The Main aim of this trial is to add further drug to keep patients free from pain. Patients are enrolled with the ages of 18-60 yrs where randomization takes place into 3 groups (only drug, drug +drug, placebo) and result will be monitored within 6 months. Hence treatment can be done by migraine mavens.

What is migraine...?

Headache gives severe pulsing sensation on one side of the head with nausea, vomiting. Migraine attacks are due to more sensitivity of light, sound, flavours and tastes (symptoms and severity differ from each individual) and it lasts from hours to days.     

                                                       

 It undergoes 4 stages:

1) Prodrome-it predicts migraine before 1 or 2 days with the symptoms of increase in urination, neck pain and constipation.(Lasts from hours to days).

2) Aura –unpredictable symptoms of nervous system (5-60 minutes).

• Ø  Classic Migraine - with aura.
• Ø  Common Migraine - without aura.

Example - loss of vision, difficulty in speaking, numbness on side of the face.

3) Headache –insomnia, throbbing (one or both sides of the head lasts from 4- 72 hrs).

4) Postdrome - after attack of migraine sufferers feel exhausted.(24-48 hrs).

Types  of migraine - Menstrual, abdominal, ocular, basilar and hemiplegic.

AWARENESS OVER COVID-19

This study comes under Interventional Clinical Trial and this can be organized through social networks (such as Facebook, youtube, webex....)

OBJECTIVE –The physicians are more attentive towards migraine sufferers for online awareness intervention trail.

 200 subjects are enrolled for the Clinical Trial (open label studies). Start date at 20-3-2020 and the approximate outcome date may be December 2020 (approx).

INCREASE IN HEALTH EVALUATION OF MIGRAINE SUFFERERS

OBJECTIVE-Prevention of Migraine  

This study comes under Interventional Clinical Trial, between 18-65 yrs subjects should be enrolled for this trail (open label) conducted at USA through REDCap app can be installed by using mobiles. Here participants are randomized towards MUC (monitored usual care –less than 2 or 3 days for acute migraines)and PMR(progressive muscle relaxation therapy-20 minutes per a day).

INNOVATIVE MEDICATIONS IN PHASE 3 TRAIL

 This study comes over phase 3 trails, where 1000 subjects were enrolled and picked through randomization (double blind studies) conducted at Unites states. Gepant drugs (Ubrogepant & Remegepant) targets Calcitonin gene- related peptide and blocked the CGRP pathway to reduce migraine pain.

REMOTE SURVEILLANCE AND GOALS TO TREAT MIGRANE BY USING DIGITAL BIOMARKERS

OBJECTIVE- Patients should not show any side affects after the treatment (nothing but safety, efficacy)

It undergoes interventional clinical trail, patients should be enrolled with the age of above 18 yrs (10-15 MIGRAINE DAYS/ MONTH). Migraine specifications can be recorded with various intervals using King Devick test 3 times per a day).

COMPARISION OF PROLACTIN LEVELS IN WOMEN (HEALTHY SUBJECTS AND MIGRAINE SUFFERERS)

OBJECTIVE- Determination of new medications for the treatment of Migraine.

It undergoes interventional clinical trial, Around 18-50 yrs female subjects can be enrolled for this trail and should report about their routine menstrual cycle so that it predicts. Usage of current medications should be known. Example of drugs affects Prolactin levels –Haloperidol, Clozapine.

PERFORMANCE OF TINTS OR THIN FILMS ON GLASS EYE LENSES

OBJECTIVE - Reduces the sensitivity of light in chronic migraine sufferers.

It undergoes interventional clinical trial. Where, 18-80 yrs migraine sufferers can be enrolled, from the last 4 months subjects should use same medication without any disturbance and should meet the international headache society.

Drugs affects the eye are chloroquine and ethambutol.
